About the Role
Gramaze Healthcare is a registered, technology-driven home healthcare organisation and digital coordination platform based in Nigeria. We specialize in connecting families, qualified caregivers, and medical professionals through integrated technological workflows, ensuring clinical care delivery is structured, transparent, and highly personalized. We are currently expanding our compassionate team and have 4 open slots for dedicated Palliative Care Professionals.
Key Responsibilities- Deliver high-quality, patient-centered palliative care to patients in their homes, ensuring comfort, dignity, and pain management.
- Collaborate with multidisciplinary medical teams and families via our digital coordination platform to monitor patient progress.
- Maintain accurate and transparent clinical documentation of care plans and daily health observations.
- Provide emotional and psychological support to patients and their families navigating terminal or chronic illnesses.
- Adhere strictly to professional nursing and caregiving standards, protocols, and safety guidelines.
- Possession of a valid Medical and Dental Council of Nigeria (MDCN) license or a recognized care-giving certificate.
- Proven experience in palliative care, nursing, or professional caregiving environments.
- Strong interpersonal, empathy, and communication skills for interacting with vulnerable patients and their families.
- Ability to work independently and utilize digital healthcare tools effectively.
