Key Responsibilities- Assess each patient's unique condition by combining data from medical diagnoses, personal interviews, and family consultations.
- Develop comprehensive care plans to help patients adjust to medical diagnoses and transition smoothly from the healthcare system.
- Provide targeted emotional and psychological support based on health status, personal strengths, and emotional state.
- Research local support groups, community networks, and healthcare resources to determine the best fit for individual patients.
- Evaluate the effectiveness of services and resources provided to patients to ensure continuous improvement in care.
- Follow up consistently with patients throughout diagnosis, treatment, and recovery to monitor mental and emotional health.
- Maintain current knowledge of local healthcare resources and relevant medical research to provide optimal recommendations.
- Bachelor's or Master's degree in Social Work or a related field with valid professional certification.
- Excellent communication skills, including the ability to explain complex medical concepts to patients and families in clear, understandable language.
- Compassionate, empathetic demeanor with the ability to connect with patients of all ages and medical conditions.
- Strong organizational, documentation, and time-management skills to handle multiple patient cases effectively.
