About the Role

Key ResponsibilitiesAs a Community Health Nurse at Gramaze Healthcare, you will be instrumental in delivering top-tier medical care directly to patients in their homes. Your daily duties will include:
- Providing comprehensive nursing care and health assessments for patients in home settings.
- Administering medications, monitoring vital signs, and executing personalized care plans designed by medical professionals.
- Educating patients and their families on disease prevention, health management, and post-treatment care.
- Collaborating with our digital coordination platform to log updates, track patient progress, and ensure seamless communication across the care team.
- Maintaining accurate clinical records and adhering strictly to healthcare compliance and safety standards.
To be considered for this exciting opportunity, candidates must meet the following criteria:
- Possession of a valid Medical and Dental Council of Nigeria (MDCN) license, or a recognized caregiving certificate.
- Demonstrated experience in community health, nursing, or professional caregiving.
- Strong interpersonal and communication skills with a compassionate approach to patient care.
- Ability to adapt to technology-driven healthcare workflows and digital reporting tools.
- High level of professionalism, reliability, and emotional intelligence.
