About the Role
JCP MAX Medical Diagnostics is a leading healthcare diagnostics provider offering comprehensive medical investigations across 2,500+ laboratory tests, advanced ultrasound services (2D, 3D, 4D), Color Doppler scans, X-ray imaging, and electrocardiography. We are recruiting a compassionate Community Health Extension Worker to join our growing team and make a meaningful impact on community health.
As a Community Health Extension Worker, you will serve as a vital link between our organization and the communities we serve, providing health education, screening services, and preliminary assessments while promoting preventive healthcare practices.
Key Responsibilities- Conduct community health education and awareness programs on disease prevention and wellness
- Perform preliminary health screenings and basic vital sign assessments
- Facilitate health referrals and coordinate patient access to diagnostic services
- Maintain accurate health records and compile community health data
- Provide health counseling and support to community members
- Collaborate with healthcare teams to implement health initiatives
- Conduct follow-up visits and monitor patient compliance with healthcare recommendations
- Minimum National Diploma (ND) or Higher National Diploma (HND) in Community Health, Nursing, or related health field
- Valid registration with relevant professional body (CHEWA, NMCN, or equivalent)
- Proven experience in community health or primary healthcare settings
- Strong communication and interpersonal skills
- Ability to work independently and within team environments
- Knowledge of health promotion and disease prevention strategies
