Key Responsibilities- Review and optimize ICU medication orders for dosing accuracy, drug interactions, contraindications, and organ function.
- Participate in multidisciplinary ICU rounds to provide evidence-based pharmaceutical care for sepsis, shock, and complex critical conditions.
- Monitor high-alert medications, manage therapeutic drug levels, and support robust antimicrobial stewardship programs.
- Identify, prevent, and document medication errors, adverse drug reactions, and near misses to ensure patient safety.
- Conduct accurate medication reconciliation during transitions of care and provide expert drug information to patients and clinical teams.
- Maintain compliance with hospital policies, regulatory standards, and quality improvement initiatives within the pharmacy service.
- Bachelor's Degree in Pharmacy (B.Pharm/PharmD) from a recognized institution.
- Current registration and valid practicing license with the relevant Pharmacy regulatory authority.
- Minimum of 3 years of post-qualification clinical pharmacy experience in a hospital, ICU, or acute care environment.
- Postgraduate training or certifications in critical care pharmacy, antimicrobial stewardship, or medication safety are distinct advantages.
