About the Role
The Quality Improvement Assistant plays a vital role in strengthening maternal, newborn and child health (MNCH) services across assigned health facility clusters in Ebonyi State. You will support the HARMONY Care project—implemented by Jhpiego with support from The Church of Jesus Christ of Latter-day Saints—by ensuring pregnant women, mothers, newborns and infants receive appropriate, timely and continuous care through improved referral systems, quality monitoring and community-facility coordination.
Key Responsibilities- Track community-to-facility and facility-to-facility referrals using approved tools, confirming clients reach receiving facilities and receive required services
- Support functional referral and counter-referral mechanisms with timely communication of outcomes and follow-up actions
- Monitor missed appointments and clients at risk of loss to follow-up, ensuring timely re-engagement through approved channels
- Conduct routine supportive monitoring visits across assigned facilities and communities using approved checklists and dashboards
- Support facility teams in implementing quality improvement actions related to service readiness, respectful care, documentation and client experience
- Strengthen coordination among supported facilities, community health workers and community leaders
- Collate and update client-tracking, referral and quality improvement tools; conduct data completeness checks
- Maintain facility directories, activity trackers and action logs for assigned clusters
- Contribute to workplan development, field scheduling and activity tracking
- Bachelor's Degree, Higher National Diploma or professional qualification in Nursing, Midwifery, Community Health, Public Health or related health field
- Minimum three years post-qualification experience in MNCH, community health, quality improvement or health facility strengthening
- Demonstrated experience with client referral, follow-up, continuity of care and demand generation
- Experience in programme monitoring, data collation and field documentation, preferably within Nigerian government health structures
- Current professional registration or licence where required
- Strong interpersonal, organisational and communication skills
- Proficiency in Microsoft Office; familiarity with Kobo Toolbox or ODK is advantageous
- Willingness to undertake regular travel within Ebonyi State
