About the Role
Gramaze Healthcare is a registered, technology-driven home healthcare organisation and digital coordination platform based in Nigeria. We specialize in connecting families, qualified caregivers, and medical professionals through integrated technological workflows, ensuring clinical care delivery is structured, transparent, and highly personalized. We are currently looking for compassionate and dedicated professionals to fill four slots for Hospice Care Professionals.
Key Responsibilities- Provide compassionate end-of-life and palliative care to patients in home-based settings.
- Administer medications, manage pain, and monitor vital signs in accordance with established care plans.
- Collaborate with multidisciplinary teams and family members via our digital coordination platform to ensure seamless care delivery.
- Maintain accurate and transparent digital records of patient care, symptoms, and medical updates.
- Offer emotional and psychological support to patients and their families during challenging times.
- Ensure patient comfort, dignity, and safety are prioritized at all times.
- Must possess a valid Medical and Dental Council of Nigeria (MDCN) license OR a recognized caregiving certificate.
- Proven experience in hospice, palliative, or home healthcare services.
- Strong empathy, patience, and exceptional interpersonal communication skills.
- Ability to adapt to technology-driven healthcare workflows and digital platforms.
