About the Role
EHA Clinics is a world-class primary healthcare provider committed to delivering quality and affordable services directly to communities. As a Community Health Extension Worker (CHEW) with our REACH program, you will be instrumental in closing healthcare gaps by bringing essential health services to patients in clinics, homes, and through innovative digital solutions.
Key Responsibilities- Deliver community and home-based healthcare services with emphasis on prevention, health promotion, and basic curative care within assigned clusters
- Provide culturally appropriate health education on chronic disease prevention, nutrition, and physical activity to vulnerable populations
- Conduct community outreach and awareness campaigns on general health, lifestyle changes, and health risks
- Assist patients in navigating healthcare systems and accessing social services and health benefits
- Manage care transitions for vulnerable patients and reduce social isolation through consistent engagement
- Maintain accurate client records, complete administrative documentation, and update databases through weekly checks
- Determine eligibility and enroll qualified individuals in health insurance programs
- Support REACH clinic activities and provide prompt assistance for comprehensive patient care
- Valid Certificate of Training as a Community Health Worker (CHEW)
- Active practice license from the Community Health Practitioners Registration Board of Nigeria
- Minimum of 1 year relevant experience in community health or healthcare delivery
- Fluency in at least one local Nigerian language and proficiency in written and spoken English
- Strong interpersonal and communication skills with cultural competence
