About the Role
The Center for Clinical Care and Clinical Research is seeking dedicated professionals to serve as Client Navigators for the BRIDGE-N Project across Kano, Jigawa, Sokoto, Kebbi, and Zamfara States. In this vital role, you will empower clients to navigate integrated health services, maintain scheduled appointments, and ensure continuity of care across HIV, Tuberculosis, MNCH, and malaria interventions.
Key Responsibilities- Support client navigation and linkage to relevant HIV, TB, malaria, and MNCH services within the approved project scope.
- Follow up with clients on scheduled appointments to support timely return to care.
- Promote continuity of treatment and care, documenting all follow-up actions accurately.
- Coordinate closed-loop referrals between services and facilities to confirm service access.
- Maintain secure client records while strictly protecting patient confidentiality.
- Collaborate effectively with facility and community healthcare teams.
- Degree or diploma in Nursing, Social Work, Psychology, Public Health, or Community Health Extension Worker (CHEW) qualification.
- At least one year of relevant professional experience, or 3-5 years of direct experience in a hospital setting.
- Fluent communication skills in both English and Hausa (residence in the applied state is highly preferred).
